We respect each person’s dignity, culture, preferences, and right to make choices. Service decisions are based on individual needs, program eligibility, authorized services, and our ability to provide appropriate support. Employment decisions are based on job-related qualifications and applicable requirements.

We consider requests for reasonable accommodations and communication assistance. We do not tolerate harassment or retaliation against anyone who raises a concern or requests an accommodation.

Website privacy notice

Information you provide. Our support and referral forms collect the contact information, location, service interests, and message you enter. Our employment application also collects your work preferences, experience, and qualifications.

How we use it. We use submitted information to respond to requests, coordinate next steps, review employment applications, and contact you. Website submissions are stored in the site’s database and are available through an access-controlled staff inbox. Website hosting systems may also process technical information needed for security and operation.

Information to leave out. These forms are for initial inquiries and employment applications. Do not submit medical records, diagnoses, Medicaid numbers, Social Security numbers, financial account information, or another person’s identifying details. Our office can explain how to provide any required documents through an appropriate process.

Your choices. Required fields are marked on each form. You can contact the office instead of submitting a form. To request access, a correction, or deletion of information you submitted, contact info@opulentresidentialservices.com. We will review your request, subject to applicable recordkeeping obligations.
